Form type

If you want to confirm someone is bankrupt, ask them for the trustee's details and AFSA administration number.

One of your obligations when bankrupt is to keep your trustee informed of any changes in your income or employment.

During bankruptcy, you must stay in contact with your trustee and inform them when your circumstances change.

A form to submit a proposal to creditors without holding a meeting.

Form 7 - statement of claim. To enable the trustee to assess your ability to vote and the value of your vote you need to complete this form.

Form 39 - application for determination of trustee remuneration. Trustees can use this form to apply to the Inspector General to determine remuneration.

Form 38 - notice of meeting of creditors. Trustees must use this form to tell creditors of a creditor meeting.

Form 37 - Record of persons present at meeting of creditors. Bankruptcy Act 1966: s75-145 Insolvency Practice Rules (Bankruptcy) 2016

Form 36 - appointment of proxy. Complete this form if you wish to appoint a proxy to vote at a meeting of creditors and provide it to the trustee.
