Unclaimed moneys claim form

If you believe you are entitled to money from a previous or current bankruptcy, personal insolvency agreement or debt agreement, you can use this form to apply to claim that money back.

Examples of unclaimed money:

• Money a creditor is entitled to who has been unable to be located by a Practitioner
• Surplus funds owed to a bankrupt who has been unable to be located by a Practitioner

When you submit this form, you must have the relevant supporting documents ready to attach.
